Submit a Final Report

Completion of a final report at the end of the CIty's fiscal year in which funded activities occurred is a requirement of all Arts & Culture funding agreements. Reports are, generally, due August 31, 60-days after completion of the City fiscal year, June 30, unless otherwise stated in the grant agreement. 

Grantees who submit a late final report may be put on a temporary funding moratorium. Grantees with an outstanding report are not eligible for any City of Dubuque Arts & Culture grant programs until the report is completed and submitted.

Final reports must be submitted online via the City's form center. Final Reports will not be accepted in any other format. Grantees can access current reporting forms below:
